To complete your application correctly and in full, you will need to provide us with some documentation and evidence. It is advised that you gather all or as much of the following documentation as possible before submitting your application to avoid having to restart your application multiple times.
Documentation you WILL need to provide includes:
- Identification
- Drivers License, and
- Medicare Card or Passport or Birth Certificate.
- USI
- An alphanumeric 10 digit Unique Student Identifier.
If the address on your driver's license does not match the address you provide on this enrolment application, you will need to provide some form of proof of address. Forms of proof of address can include:
- Utility Bills (Water, Gas, Electricity, Internet, ect.)
- Bank Statements
- Centrelink Letters (with full name and address in header)
- Receipt of updated address
Any proof of address you provide must clearly show your full address and full name.
Documentation you MAY need to provide (depending on your individual circumstances) includes:
- USI Transcript.
- Details of Employment Services Provider.
- Copy of VEVO letter (visa holders only).
If you can provide any of the above information, it is ideal to have it on hand before starting this form in case it is requested as part of your application.

If you have lost or forgotten your USI, you can find it using the following link:

To download your USI transcript, you will need to log into you USI account using the link below:
https://portal.usi.gov.au/student
You will then need to navigate to "View VET Transcript" and click on the "Download Transcript" button. After following the remaining instructions, you will receive a downloaded PDF version of your USI VET Transcript that you can upload to this application.
Document Quality:
Please ensure that the documents you are uploading (either image or PDF format) are of a clear and readable quality. Some quality requirements for official documentation includes:
- Colour Copies of all Documents, not black and white (ID, USI Transcript, bills, etc)
- Clear and not blurry
- Images not too small (easy to read)
- No faded text
- No glare on glossy IDs from flash or other lighting covering important info (like names, ID numbers, and expiry dates)
- Valid (in-date) expiry dates.
If these quality requirements are not adhered to, it is likely we will have to get in contact with you for resubmission of documents.
